Our goal is to ensure every person understands their options, accesses the resources they need, and feels empowered to shape a life of dignity, independence, and connection.

Navigating Services & Accessing Resources
We help individuals and those who support them understand what public and disability-specific services are available—and how to access them. This includes guidance on programs such as:
- In-Home Supportive Services (IHSS)
- Department of Rehabilitation (DOR)
- CalFRESH
- Regional Center
- Supplemental Security Income (SSI)
- Medi-Cal
- Other community-based, disability specific supports
- Home and Community Based Services (HCBS)

Applications, Eligibility & Appeals
We assist with completing applications for services, understanding eligibility criteria, and navigating next steps when services are denied. If requested, we provide hands-on support with preparing appeals and understanding recourse options—ensuring individuals are not left to advocate alone.
Building a Path Forward
Building a Path Forward for Employment, Housing, and Education. We support connection to employment programs, accessible housing options, supported and independent living resources, and adult education or training opportunities—based on each person’s unique needs, goals, and vision for the future.
Young Adult Transition Planning
We help young adults and their families prepare for the shift from school-based services to adult systems. From identifying adult service agencies to planning for post-secondary goals, we provide a clear and compassionate roadmap for what comes next.

- We will work with you until all of the pre-enrollment requirements are completed and you are accepted into the Self-Determination. (Regional Center Service Code 099)

- We offer ongoing Independent Facilitator support to help you navigate services, funding, and choices.
